SMOKING CESSATION PROGRAM:

If you are thinking about quitting, congratulations!  Whether this is the first time you are thinking about it, or you are trying to quit again, we know quitting cigarettes can be physically and mentally challenging.

Everyone has their own reasons to quit.  Perhaps you are worried about your health; or you want to protect the health of your family.  Some people want to save money instead of needing to buy cigarettes; or maybe you just don’t like smoking anymore.

Have a plan

Regardless of your reasons to quit, having a plan and knowing what to expect can increase your chances of success.  Having supports in place may make your quit journey less overwhelming.

We are here to help you quit if you are ready

As part of the North York Family Health Team, you have access to a free program to help you quit smoking.

We can offer you

  • Counselling, techniques and resources to  help support your goals

  • Information about oral medications and smoking cessation aids

  • Access to free nicotine replacement (patches, gum, lozenges, inhalers) for up to 6 months

NURSE PRACTITIONER HOMEBOUND PROGRAM:

A Nurse Practitioner led home visiting program to provide episodic care to support NYFHT patients (residing in the North York region) who may be experiencing:

  • Mobility issues due to frailty

  • Recent discharge from hospital

  • Social determinants i.e. accessible transportation

The NP visits the patient’s home to assess, order appropriate testing, prescribe medication and navigate patients to applicable FHT and community resources as needed. The NP collaborates with the patient’s Family Physician to maintain continuity of care.

COGNITIVE BEHAVIOURAL THERAPY FOR DEPRESSION AND ANXIETY:

This 6 session program is designed to help individuals struggling with depression and anxiety develop a greater understanding of their experience and learn strategies and techniques to manage their mood, and reduce the risk of relapse.

COGNITIVE BEHAVIOURAL THERAPY FOR INSOMNIA:

Cognitive Behaviour Therapy for Insomnia (CBT-I) is a 6 session program which helps people who have been struggling with insomnia. This program includes a CBT-I and a Deprescribing component. This program helps to improve sleep quality and provides an effective alternative to sleep medication.

For those already taking a sleep medication, as part of the Deprescribing Initiative, Pharmacists will discuss the pros and cons, and offer guidance to individuals interested in gradually coming off their sleep medication safely.

OBESITY AS A CHRONIC DISEASE PROGRAM:

A lifestyle management program utilizing diet and behavioural management interventions. Patients will have a plan created based on their metabolic disease and personal preferences which will include a whole food, low carbohydrate diet within time-restricted eating windows. This program is led by Physician consultant and in collaboration with NYFHT Registered Dietitians, Pharmacists and Social Workers. Follow up with the Physician consultant may be made at 6 months if necessary, which will require another referral from the Family Physician.

MINDFULNESS FOR HEALTHY EATING:

This group program would benefit those who may be frustrated with years of dieting and are looking for a different approach. By learning health enhancing tools, participants will develop a more positive relationship with food and build on health lifestyle habits and attitudes. This 8 week interactive group program is run by a Registered Dietitian and a Social Worker and uses Intuitive Eating, Craving Change, as well as Mindfulness and Cognitive Behavioural Therapy.

MINDFULNESS BASED COGNITIVE THERAPY:

MBCT is a 10 session program to help individuals struggling with depression, anxiety, emotional distress and physical pain. Through meditation and mindfulness informed practices, individuals will learn to cultivate awareness of their inner experience in order to develop emotional resilience. The practices learned in this program help participants maintain emotional wellness and reduce depressive relapse.

MENTAL HEALTH SERVICES:

Our Mental Health team provides a holistic and team-based approach to mental health care. We support individuals experiencing difficulty with various mental health challenges such as depression and anxiety. We offer the following short-term services: individual counseling, group programs, case management and psychiatric consultation. In addition to Social Work, Case Worker support is available to navigate financial and housing needs,  as well as community resources.

HEART HEALTH PROGRAM:

Have you been told by your Health Care Provider that you have high cholesterol, high blood pressure, or a family history of premature heart disease? Join our interactive group session where participants learn how changes in nutrition, physical activity, medications, stress and other aspects of our everyday living can play a role in improving our heart health.

DIABETES EDUCATION PROGRAM:

The Diabetes Education Program supports individuals that are newly diagnosed or have been managing Pre- or Type 2 diabetes. We offer free services in both group and individual settings for diabetes management including nutrition, physical activity, blood glucose monitoring, and medication management.

COLORECTAL CANCER SURVIVORSHIP PROGRAM:

Individuals who have received treatment for colorectal cancer (stage 1, 2, or 3) at North York General Hospital will receive one-on-one care by our Nurse Practitioners, who work collaboratively with the surgical and oncology teams at North York General Hospital.
